This chronic disease management site was developed to provide information specifically to health care professionals working in chronic disease management and is intended for informational and educational purposes only.  The target audience includes general practitioners, general practice nurses and allied health professionals. 

The purpose of the site is to facilitate multidisciplinary care, thus improving outcomes for patients with chronic and complex conditions.  The site provides information about the overall services health care professionals may provide and their training and qualifications.  It uses three chronic diseases as examples to provide more specific information on allied health professional services, trigger points for referral to an AHP, and how a multidisciplinary team may be formed.

The site also provides information on improving communications between allied health professionals and general practice.
